ATTENTION: This is a form to be filled out by students who require proctoring for their tests. If you are able to come to Barstow College in either Barstow or Fort Irwin, you should do so, and you do not need to fill out this form. (Students who are close to Barstow College or Fort Irwin will not be approved for proctors except in extenuating circumstances - contact the instructor for approval before submitting this form or your request will be denied.)

In addition, filling out this form does not automatically mean your proctor has been accepted. The proctor will still need to receive instructor approval.

Barstow College has specific requirements for a person to serve as a proctor. Please be sure your proctor meets these requirements BEFORE submitting the information:

* All proctors must be librarians, military educational officers, college or K-12 instructors/teachers, professional proctors (such as our lab aides), or clergy.

* The proctor must have a professional email address and phone number. (Hotmail, yahoo, gmail, excite, aol, netscape, earthlink, verizon, netzero, etc., email accounts are unacceptable.)

* Proctor phone number cannot be a cell phone and address cannot be a home address. Phone number and test location must match.

* The proctor must have a computer with reliable internet access available in the supervised area for you to take the exam. Do not assume that the proctor has a computer - ask. All exams are on the computer. Printed/paper exams are not available.

* Regardless of profession, friends, family members, and neighbors are not suitable proctors.

Your instructor will email you with proctor approval and give you further information. You should check with your proctor the morning the final exam begins to be sure they received the information.

Finally please remember that the top box is for YOUR information and the bottom box is for your PROCTOR'S information.  You can not be your own proctor.  A proctor for this class must be unrelated to you, and be approved by the instructor.  Any costs related to proctoring are to be born by the student.  The college is not responsible for proctoring fees.
