Proctor Form

Attention all students:  Having a proctor is a requirement to offer this course. It is imperative that you secure a proctor and submit a completed proctor form.

You have until January 18, 2009 to submit a Proctor Form.  If you do not have a completed Proctor Form submitted within the given time frame, your final grade will be reduced by one letter grade.

A Proctor is used so that the Instructor can communicate with a third party to confirm that the student is participating in his/her chosen activity for a minimum of three hours per week. A proctor can be a family member, co-worker, instructor, etc.

If you have any questions regarding getting a Proctor or completing the Proctor Form, please contact the Instructor immediately.
