ATTENTION:  This is a form to be filled out by all students.  Please use the following guide when completing the form.

Barstow Area Students: Take your exam at the Barstow College Computer Labs. For proctor name, type Barstow Computer Labs.

Fort Irwin Students: Take your exam at the Barstow College Office in Fort Irwin. For Proctor name, type Fort Irwin Campus.

All Other Students: You must find a qualified proctor. Your best option is to use someone in the field of education. Your next option is a librarian. These individuals are most likely to be approved quickly. You must take your exam at an educational faculty or a library. If you need to use someone other than an education officer or librarian please contact me prior to submitting your proctor request. Always check to ensure your proctor is willing to issue you the final exam before submitting your proctor request to me.

The proctor must have a professional email address and phone. (hotmail, yahoo, excite, aol, netscape, earthlink, verizon, netzero email accounts are unacceptable). The email address should correspond to a school or business.

Under no circumstances will a friend or family member be approved as a proctor. Additionally, you will not be allowed to take your exam at an individual's home.

In addition filling out this form does not automatically mean your proctor has been accepted.  The proctor will still need to receive instructor approval.  

Finally please remember that the top box is for YOUR information and the bottom box is for your PROCTOR'S information.  You can not be your own proctor.  A proctor for this class must be unrelated to you, and be approved by the instructor.  Any costs related to proctoring are to be born by the student.  The college is not responsible for proctoring fees.
