Username and password are the same. The first four letters of your last name and the last four numbers of social security. Below is an example that you can use as a guide. Make sure your use your own social security number and your own last name for your username and password. 

EXAMPLE USERNAME: smit1234
EXAMPLE PASSWORD: smit1234
